Get the exhaust checked for excessive back pressure. Converter may be plugged or partially plugged.

If you have a hesitation then try checking some things over first. Plugs or plug wires have any issues, wide gap, high resistance or open plug wire? Sticky EGR valve? Possible exhaust restriction?...

After my post above I see you say they did do smoke tests, sorry about that. Maybe a small leak out the gas tank seal where the fuel pump seals?

Take your front brake calipers off, see if you can push the piston in. I have seen this on an S-series before when the caliper stuck. Are the calipers original to the car do you know?

It appears to be the cylinder head leaking. I have seen a few of the dohc heads crack there. You can try epoxy if you want, but I would replace the head for better long term results. Try cleaning...

Check your belt tensioner, it may be getting weak or it may be stuck, either way the belt could be loose causing it to slip.

It's actually recommended at 100k, so yes it would be worth it. It would be a good time to change the plugs and make sure no oil leaks are present from the camshaft covers.

If you go to a muffler shop be sure to have them check for back pressure in the exhaust. Does the car have any power loss issues?

That's a neat story. Those Zexel compressors were very dependable. I don't think I ever changed one of those on an S-series, unless the number of ones I changed were so low I don't remember, LOL.

Cleaning your throttle body out would be a great idea, at 215,000 miles if it's never been cleaned I guarantee it's caked up with carbon. That could be part of your stall out issue too.

Just out of curiosity, have you tried removing the front o2 sensor or checked for too much back pressure in the exhaust. I have not read all ten pages so forgive me if it's already been checked.

Double check and make sure your wires are hooked up right on the coils, if 1 and 4 are switched you will have a tip in hesitation.

You need a scan tool that will read live data, one that can show misfires even if the check engine light has not came on yet.
