I know what you are saying. There is NO clearance between the fender liner and the tensioner for a 3/8" ratchet. I tried. You would have to buy a serpentine belt tool. I may just have it changed...

I had problems with the entire control head on mine. I took it out of the dash and 'cleaned' all the switches in the head. They were pretty carboned up. Works like a champ now.

The insides wore out first because the piston moves that pad. The outer pad relies on the slider pins. Your slider pins need to either be replaced, preferable with that many miles, or cleaned up...
