I would think that if it was the belt tensioner, you'd still hear it at 2k rpm with no load.

If the fuel filter has over 120k miles or so, I'd check fuel pressure.

Might be worth checking MAP...

Fixing it yourself is pretty low cost. Haven't done it on a sedan, but have read that there is a richpin video - just a bunch of 10mm head bolts, mostly. I think you have to remove the tail light...
